ANESTHESIA DEPOSIT AND FEES
All patients are required to sign a financial agreement (see below). An anesthesia deposit must be received to secure your appointment, and full payment is due on the day of your procedure.

Anesthesia fees are based on the length of the surgery. Billing time includes an additional 30 minutes beyond the dental procedure (15 minutes before and after) to allow for safe preparation and recovery.
A final bill will be provided at the completion of your procedure. Dr. Smith currently accepts Medicaid and fee-for-service payments. Patients may use this bill to submit claims to their private insurance if desired.

Pediatric Patients (13 years old and younger)
Pediatric Fees

$1000 flat fee for every case which is 2 hours or less (anesthesia billing time).

$600 / hour billed in 15-minute increments for cases longer than 2 hours.

Mandatory $1000 deposit is required to hold the appointment. Applied to final bill. Please inquire about referral discounts.
Adult Patients (14 years old and older)

$1350 flat fee for every case which is 2 hour or less (anesthesia billing time).
