ANESTHESIA DEPOSIT AND FEES
All patients must sign the financial agreement (see below). The anesthesia deposit must be received to confirm your appointment. Full payment is required on the day of anesthesia.

The entire anesthesia fee is determined by the length of the surgery being performed. Anesthesia billing time is 30 minutes longer than dental treatment time (15 minutes added to both the beginning and end of the procedure to allow for safe induction and recovery).

The final bill is available as soon as the surgery is complete. Dr. Smith currently accepts Medicaid and fee-for-service only. This bill is provided to help patients submit to private insurance if they choose.

Alpine Dental Anesthesia fees are separate from the dental/surgery fees. All major credit cards and cash accepted. Ask about a cash discount.
Pediatric Patients (13 years old and younger)
Pediatric Fees

$1000 flat fee for every case which is 2 hours or less (anesthesia billing time).

$600 / hour billed in 15-minute increments for cases longer than 2 hours.

Mandatory $1000 deposit is required to hold the appointment. Applied to final bill. Please inquire about referral discounts.
Adult Patients (14 years old and older)

$1350 flat fee for every case which is 2 hour or less (anesthesia billing time).
